Ava Cadavra is the second character to occupy the Penthouse in The Sims 2; she arrives shortly after the arrest of Frankie Fusilli. Ava is as cunning and secretive as she is volatile, though later in the game she's shown to have mellowed out a bit.

Upon her arrival, Ava insists that the player build the Bovine Shrine for her. Once the Shrine is built, she will remain there until she checks out of the Penthouse. It isn't long before Ava convinces three sims, Ara Fusilli, Auda Sherif, and Sancho Paco Panza, to worship the Prime Heifer with her. Ara, Auda, and Sancho can be seen standing in the Bovine Shrine, locked in an endless trance as they chant mindlessly to awaken the bovine spirit. Ava will send the player on a few small errands, asking that they bring her the Mystic Cowbell and her favorite lipstick shade, Rotting Eggplant, before remarking that they seem unhappy and that they should "buzz off."

At this point, the player will receive a mysterious phone call instructing them to go to the Desert at night and find the Pyramid. When the player locates the crypt in the Desert, Horus Menhoset IX will appear and ask them to unearth his sarcophagus from the back of the Oasis. Once the Sarcophagus is in the player's possession, Horus Menhoset IX will instruct the player to put it "somewhere only you can reach," this place being the Manager's Suite. Shortly after, he sends the player to ask the residents of Strangetown for the whereabouts of the Branding Iron of Udder Destruction. If the player talks to Jebediah S. Jerky, he will tell them that it's in the jail, as he and Honest Jackson had accidentally left it there. After retrieving the branding iron, Horus Menhoset IX tells the player to brand the Prime Heifer, thereby destroying it. When the player uses the Branding Iron to destroy the icon, Ava will grow enraged, screaming at the player that they can expect no payment from her and storming out of the Hotel (though not before rushing back up to the Penthouse to "redecorate" with flames).

If the player hasn't purchased the permit for the Rat Cave at this point, the Penthouse will not be on fire when the player and Jeb arrive. However, if the player has already built the Rat Cave, the Penthouse will be on fire. Jeb will also inform the player that Ava left her wallet behind, allowing them to keep the §3,500 inside. Since this is the cost of the Rat Cave building permit, the Rat Cave is free no matter the player's choices. Immediately after this scene ends, Tristan Legend will ask the player to meet him in the basement so that he can show them the Rat Cave,

After her departure, Ava will return to the Strangetown Hotel as a regular guest. She assures the player that she's turned over a new leaf and merely wants a place to lay her head while she works on her poetry. If the player becomes best friends with Ava, she'll share her secret with them: she used to be a ballerina, but was kicked out for being too tall.

If the player's sim is female, they will be able to unlock Ava Cadavra's outfit upon the completion of the game's main storyline.
